Rime of the Ancient Mariner by Samuel Taylor Coleridge, the song was done by Iron Maiden, pretty much follows the poem really well.

Evangeline A Tale of Acadie is a poem, published in 1847, by the American poet Henry Wadsworth Longfellow.
Evangeline is a 1981 album by Emmylou Harris with the song on track 5. but I don't know if that is directly related.
The French verson is.

Leonard Cohen is a great songwriter who was a Beat poet. Most of his songs are poems set to music. For example, one of his better known songs is "Bird on a Wire", which has been recorded by Johnny Cash, K.D. Lang and many others:
like a bird on a wire
like a drunk in a midnight choir
I have tried in my way to be free.
like a baby stillborn
like a beast with his horn
i have torn everyone
who reached out to me.
etc.
Check him out.

star spangled banner was written as a song (to a British drinking song)
Highwayman is a good Irish lyrical poem but hasn't really hit mainstream at least in america

"Desolation Row" by Bob Dylan. yeah, it's one of his songs, but it must have started out as a poem.
